In order to be eligible for volunteer work in the FBC Baseball Youth Leagues, you must consent to the following statment and answer the questions below. A background check will be completed for all volunteers to ensure the safety of the players in the league.

The cost of the background check is $8.00. This must be paid by the volunteer.

As a condition of volunteering, I give the First Baptist Church permission to conduct a background check on me, which may include a review of sex offender registries, child abuse and criminal history record. I understand that if accepted, my position is conditional upon the league receiving no inappropriate information on my background. I hereby release and agree to hold harmless from liability the First Baptist Church Youth Baseball Leagues, the officers, employees, and volunteers thereof, or any other person who may provide such information. I also understand that regardless of previous appointments, the First Baptist Church Youth Baseball League is not obligated to appoint me in a volunteer position. If appointed, I understand that prior to the expiration of my term, I am subject to suspension by the Baseball Commissioner, and removal by the Rules Committee by the First Baptist Church Youth Baseball League for violation of league policies or principles.
